I've seen only a few of these over the years, and they still remain a sought after precision indicator! The indicator has been checked and verified to be in good working order, no wear noted, It is clearly marked "THE L.S. STARRETT CO. ATHOL MASS. U.S.A. No. 564 PAT. NO. 1958028" on the indicator. This model is design to attach to a surface gauge round rod, or even a lathe tool holder, as is basically has a sturdy washer like threaded side clamp that squeeze the rod tightly and hold the indicator in place just fine. This is a very nice, hard to find model from the 1920's-50's era!  Also included in this offering is the original cardboard box (some tape on the exterior mostly with a matching advertising/instruction brosure folded up inside the box.  The box and brosure both have storage wear, but the brosure is very rare and is helpful in understanding how this indication was designed to be used.  
The wooden box is nicely made, very nice lap joints in the corners, some age darkening, but clean with a good label on one end.  No damage noted, other than a few small chips off the top corners at the mouth of the sliding cover.
